This patch release fixes an urgent codex-acp permission request handling issue. When allow_always was selected automatically, codex-acp only matched requests against already-approved command prefixes, so unapproved commands returned user abort and could not run. The app now selects allow_once so the current permission request can proceed as a one-time approval. Fixed - Fixed ACP Agent permission request auto-handling selecting allow_always, which triggered codex-acp's approved-prefix matching limit and caused unapproved commands to return user abort instead of running
